Acute medullary infarct secondary to the probable extracranial vertebral artery dissection.

Findings
Right half of medulla shows a Diffusion restricted lesion which is hyperintense on T2 and T2 FLAIR of size less than 20 mm.
CT and MR Angiography show diffuse narrowing of distal V2 segment of right vertebral artery from C2 to C5 levels with mural thickening giving a " string sign." The V1, proximal V2 ,V3 and V4 segments are normal. Left vertebral artery is normal.
Anterior circulation vessels are normal.
Diagnosis
Acute lacunar infarct in right side of medulla (6 hours - 4 days) probably secondary to vertebral artery dissection of distal V2 segment at C2-C5 level.
