Ruptured Ovarian Dermoid

Findings
Gross hypodense collection of fluid attenuation value seen in supra & inframesocolic compartments of peritoneal cavity, more so in pelvis, measuring ~ 2 to 3 litres.

Irregular areas of hyperdensity of blood attenuation value seen in peritoneal cavity, in lower aspect of paracolic gutters, both iliac fossae & pelvis measuring ~ 250 cc. Few areas of fat density also seen in pelvis.

Irregular heterogenous soft tissue density lesion with hypodense areas of fat attenuation within & peripheral hyperdense rim measuring ~ 5 x 4 x 5 cm seen in right adnexa with adjacent stranding. No calcification. No invasion of adjacent structures. Lesion could not be separated from right ovary. Left ovary also could not be visualized adequately.
Diagnosis
Gross ascites & Large pelvic hematoma/ clot with few fat density areas in peritoneal cavity.
Irregular heterogenous soft tissue density lesion with hypodense areas of fat attenuation in right adnexa as described - Likely Ovarian dermoid.
- Possiblity of Ruptured Right Ovarian Dermoid to be considered.
