Laryngeal Rupture

Findings
Xray: Linear lucencies along the neck soft tissues & along cardiac borders.

Neck: Discontinuity seen in anterior aspect of larynx in the supraglottic region just above the level of false vocal cords in the region of anterior commissure measuring ~ 8 mm in depth & 3 mm in width communicating with the neck spaces.

Pockets of air are also seen in right paraglottic space & pyriform sinus dissecting in between the inferior thyroid & cricoid cartilages causing moderate narrowing of airway.

Multiple pockets of air density seen in superficial & deep spaces of the neck on both sides with thickening of fascias & also in suboccipital & paravertebral region on right side.

Pockets of air density seen in pre vertebral space extending into the mediastinum.

Edema of aryepiglottic folds seen on both sides causing moderate narrowing of lumen.

Multiple pockets of air also seen in superficial & deep fascial planes of neck region on both side.

Thorax: Multiple pockets of air density seen in superior, anterior, middle & posterior mediastinum, tracking from the neck region.

Surgical emphysema seen in anterior Chest wall on left side.
Diagnosis
Discontinuity in anterior aspect of larynx in the supraglottic region in the region of anterior commissure as described.
Multiple pockets of air density seen in superficial & deep spaces of the neck on both sides.
Moderate narrowing of airway due to mass effect.
Pneumomediastinum.

- Features are suggestive of Laryngeal rupture with Pneumomediastinum & extensive Surgical emphysema.
