Findings
Plain CT showed a well defined hyperdense lesion centred in the left basal ganglia region (1A). MR Imaging showed a well defined lobulated mass lesion involving the left capsuloganglionic region The lesion is heterogeneously hyperintense on T1WI and T2WI ( Fig 1 B,C) .
Cystic areas are well appreciated in the medial aspect of the lesion on T2WI ( Fig 1 C). Areas of hyperintensity( 1B) and susceptibility blooming are noted on T1WI and SWI respectively ( IB , 1D) .
The most part of the lesion appear hypointense on T2 , moderately hyperintense on DWI( IC, IE) , shows heterogeneous post contrast enhancement( IF) , elevated Choline peak on MR spectroscopy (IG) and no significant increase perfusion on MR DSC perfusion ( 1H).
Diagnosis
Possibilities considered on imaging was basal ganglia germinoma, Supra tentorial ependymoma and Glioma.
