Discoid Medial meniscus with complex tear and parameniscal cyst.

Findings
There is complete discoid configuration of Medial meniscus with large chronic high-signal intensity complex Grade III tear demonstrating both horizontal and vertical components. In addition, a small multilocular fluid-signal intensity cystic lesion is seen along peripheral torn margins of posterior horn and meniscal body, consistent with parameniscal cyst. It measures upto 4mm in thickness. The cyst exerts mild mass effect over the adjacent Medial collateral ligament.

A subtle intermediate signal intensity chronic horizontal Grade III tear involving body and posterior horn of Lateral meniscus is also noted.

The presence of diffuse soft tissue edema along Medial collateral ligament and inferior Medial patellofemoral ligament is indicative of Grade I sprain.

Moderate intracapsular joint effusion with fluid-filled distension of suprapatellar bursa is seen.

A small fluid-filled midline outpouching of posterior joint capsule is seen with defect measuring 2-3mm.
Diagnosis
• Discoid Medial meniscus with Grade III chronic complex tear and small multilocular parameniscal cyst.
• Grade III horizontal tear of Lateral meniscus.
• Grade I sprain of Medial collateral and inferior Medial patellofemoral ligaments.
• Small midline outpouching of posterior joint capsule – concern for old capsular tear.
• Moderate knee joint effusion.
