Patellar tendon-lateral femoral condyle friction syndrome

Findings
There is mild swelling and increased PDFS signal within the Hoffa's fat pad located between the proximal patellar tendon and lateral femoral condyle, suggestive of patellar tendon-lateral femoral condyle friction syndrome (Hoffa's fat pad impingement). The suprapatellar / quadriceps fat pad appear normal.


Additionally, mild focal irregularity and fibrosis of Medial patellar retinaculum near its patellar attachment with a small ovoid osseous fragment of size 2-3mm is seen embedded within retinaculum fibers, suggestive of old avulsion fracture.
Diagnosis
Patellar tendon-lateral femoral condyle friction syndrome (Hoffa's fat pad impingement) likely predisposed by pre-existing High-riding patella (patella alta).
