A rare case of spinal epidural Ewing’s sarcoma : MRI Features

Findings
A lobulated, fairly-demarcated soft-tissue mass was detected in the epidural space of lower spinal canal (L5 – S3) with right paraverebral and paravertebral extension, along L5/S1 – S2/3 neural foraminae, compressing the right sacral plexus and sciatic neurovascular bundle. Chronic denervation edema with mild atrophy and fatty replacment of the muscles of right gluteal region. Irregular lytic destruction of right sacral ala was noticed with areas of trabecular remodelling. The mass was isointense to the skeletal muscles on T1 W and slightly hyperintense on T2 W and T2 W Fat saturated sequences. The mass exhibited heterogenous and moderate enhancement with areas of restricted diffusion. Smooth widening of L5/S1 – S2/3 neural foraminae was observed. Few subcentimetric patches of enhancing foci in the right posterior temporal and parietal white matter without perilesional edema were observed in brain---metastasis.
