Degenerative joint disease of the knee joint with mucoid degeneration of ACL and PCL

Findings
Moderate knee joint effusion with fluid distending the medial and lateral recess of the suprapatellar bursa. Synovium is thickened with. A sub-centimeter sized well corticated loose body is seen posterior to the posterior cruciate ligament

PD FS and T2 hyperintensity is seen in the middle and distal fibers of anterior cruciate ligament (ACL). The ligament shows mild to moderate thickening. This represents mucoid degeneration. The alignment of the ACL is normal. The posterior cruciate ligament also shows similar changes with no evidence of tear.

The medial and lateral collateral ligaments are normal.

Globular intra-meniscal FS PD hyper intensity is seen contained within the posterior horn of the lateral meniscus. This is indicative of grade I degeneration.

Ill-defined FS PD hyperintensity is seen involving the posterior horn and body of the medial meniscus. The signal intensity is invading the inferior margin of the meniscus and is suggestive of grade III degeneration.

Subchondral cysts are seen in the intercondylar regions of both femur and tibia. These are likely to be degenerative in origin.

Osteophytes are seen on medial and lateral femoral and tibial condyles. Inferior patellar beaking and spiking of tibial intercondylar eminence is seen.

Subchondral insufficiency fracture of the medial tibial condyle is seen.

There is diffuse thinning of the articular cartilage over medial and lateral tibial and femoral condyles. Changes of grade IV Chondromalacia are seen at the medial and lateral patellar facets.

Mild lateral patellar tracking is seen.

The rest of the visualized bones reveal otherwise normal signal intensity.

The medial and lateral patellar retinacula are normal.

Mild edema is seen in the Hoffa’s fat pad.

The popliteal muscle and tendon appear normal. The quadriceps tendon appears normal.

A well-defined lobulated cystic lesion measuring 26 x 11 mm is seen in the soft tissues on postero-medial aspect of distal femoral condyle.


Diagnosis
1. Moderate knee joint effusion.
2. Mucoid degeneration of the anterior and posterior cruciate ligaments.
3. Grade I degeneration of the posterior horn of the lateral meniscus.
4. Grade III degeneration of the posterior horn and body of the medial meniscus.
5. Osteophytes on medial and lateral femoral and tibial condyles. Inferior patellar beaking and spiking of tibial intercondylar eminence is seen.
6. Subchondral insufficiency fracture of the medial tibial condyle.
7. Degenerative joint disease involving tibio femoral and patella-femoral articulation.
8. Grade IV Chondromalacia at the medial and lateral patellar facet.
9. Mild lateral patellar tracking.
Features are consistent with degenerative joint disease.
