Mucinous neoplasm of ovary

Findings
A large cystic non-enhancing (HU on plain 34 and HU on contrast 35) abdomino-pelvic mass is seen with multiple foci of calcifications within. Few internal septae are seen. It measures 20x13.4x14.1 cm in axial dimensions. Adjacent peritoneal fat stranding and nodularity is seen – suspicious for involvement.

Minimal fluid is seen in right iliac fossa beneath anterior abdominal wall.

There is displacement of small bowel loops to the periphery.

Compression of one of the ileal loop is seen with dilatation of proximal jejuno-ileal loops, which measure upto 3.1cm in diameter.
Diagnosis
Predominantly cystic abdomino-pelvic mass with multiple foci of calcifications and septations within. Findings are suggestive of mucinous neoplasm of ovary in view of adjacent peritoneal involvement and ascites, malignant potential to be considered.

Small bowel obstruction – likely subacute.
