Findings
Multiple well defined conglomerated cysts of varying sizes are seen epicentered in the region of third ventricle. Together they measure approximately 5.7x5.2x5.9 cm. They are completely suppressed on FLAIR images. No restricted diffusion is seen within.No enhancement is seen on post contrast sequences. Extension is seen to fourth ventricle through aqueduct of sylvius. Extension to suprasellar and perimesencephalic cisterns is seen. Infiltration of right gangliocapsular region and right hemimidbrain is seen. Mass effect is seen in the form of upstream hydrocephalus and periventrciular ooze. Compression of pons, midbrain and cerebellar vermis is seen.

Few discrete subcm sized cysts are also seen in left hemimidbrain and right hemipons.
Diagnosis
Overall findings are suggestive of benign cystic etiology. Diagnosis of racemose NCC is favored.

Septated arachnoid cyst is less likely differential in view of small discrete lesions in brainstem apart from conglomerated intraventricular multicystic mass.
