Findings
CT Brain study reveals a well defined rounded cystic lesion measuring 4.5x5 cm in the left fronto-parietal lobe. The lesion has a well defined smooth hyperdense rim and internal hypodense contents of fluid attenuation (10-20 HU).
There is a thin hyperdense undulating linear membrane noted within the lesion, suggestive of a detached germinal membrane.

On post-contrast study, there is peripheral smooth rim enhancement noted. There is also adjacent vasogenic edema noted along with mass effect in the form of compression of the ipsilateral lateral ventricle and midline shift of 1 cm to the right side.

MR Brain study reveals a well defined lesion appearing hyperintense on T2 and hypointense on T1 and FLAIR having a T2 hypointense peripheral rim and also a T2 hypointense undulating internal membrane in the left fronto-parietal lobe.
On DWI, no significant diffusion restriction is noted in the lesion. On SWI, no susceptibility artefacts/blooming is noted.
There is adjacent vasogenic edema present. There is also mass effect in the form of compression of the ipsilateral lateral ventricle and midline shift of 1 cm to the right side. The lesion measures 4.5x.5x5 cm in CS and CC dimensions.
Diagnosis
CT and MR study features are suggestive of cerebral hydatid cyst with associated vasogenic edema, mass effect and midline shift as described.

Adv: Clinical correlation and follow up for secondary infection/rupture of cyst.
