Findings
T2 & FLAIR hyperintense and T1 hypointense areas with generalized decrease in the volume of the left cerebral hemisphere noted suggestive of gliosis due to old vascular / parenchymal insult with ex-vacuo dilatation of the left lateral ventricle and adjacent cisternal spaces.

There are seen dilated, tortuous signal voids in the bilateral basal cisterns suggestive of arterial collaterals.

T2 hyperintense areas noted in the right centrum semiovale and internal capsule extending along the descending white matter tracts in the brainstem and bilateral dentate nucleii which shows restriction on DWI sequence.
Diagnosis
Imaging features suggestive of Gliosis with hemiatrophy of the left cerebral hemisphere, possibly sequeale of vascular insult - likely Moya-moya disease
