Right coronary artery arising from left cusp

Findings
Dominance: Right
There is ectopic origin of the RCA from the left coronary cusp, with an interarterial course between the aorta and pulmonary trunk (malignant course).The proximal RCA stent shows intimal hyperplasia, with >70% stenosis. There is focal soft plaque in proximal PDA causing 30-40% stenosis. Few non-stenotic calcified plaques are seen in proximal and mid RCA. The PL shows no significant abnormality.
• The LM is normal.
• The proximal LAD shows an eccentric soft plaque, with <25% stenosis. There is an 11mm eccentric mixed plaque with 30-40% stenosis starting from the origin of D1. The rest of LAD and diagonals show no significant abnormality.
• The proximal OM1 shows eccentric calcified plaques with <25% stenosis. The rest of CX, ramus, and OMs show no significant abnormality.
