Findings
CT Brain Day 1(non Contrast and Contrast) shows hypodensity involving the right frontal, temporal and parietal lobe, basal ganglia extending into the corona radiata and centrum semiovale. Associated with sulcal and gyral effacement swelling. No midline shift. No haemorrhage. Appearances are consistent with an acute right MCA infarct.

Linear fat density (-33 HU) suggestive of embolus is seen in right MCA M1 and M2 segments. The appearances are consistent with fat embolism.

Delayed opacification of right internal carotid artery seen.

Ct Brain Day 3 post embolectomy consistent with right MCA infarct. Previously seen Linear fat embolus not visualised.
Diagnosis
Acute Right MCA territoy infarct, probably secondary to Fat embolism.
