Cerebral hemi atrophy with crossed cerebellar diaschisis.

Findings
There is diffuse atrophy of left cerebral hemisphere with widening of left lateral ventricle, ipsilateral shift of inter ventricular septum & diffuse T2 hyperintensity in white matter.
Left hippocampus shows evidence of atrophy with gliosis.
Atrophic changes are also seen in ipsilateral thalamus & cerebral peduncle with diffuse atrophy & gliosis in right cerebellar hemisphere.
Left side of skull vault is thickened, compared to left side.
Diagnosis
Left cerebral ischemic hemi atrophy with ipsilateral mesial temporal sclerosis, compensatory calvarial changes (Dyke-Davidoff-Masson Syndrome) and crossed cerebellar diaschisis.
