Penile Doppler
Technique

High-resolution B-mode ultrasound examination and Doppler was performed with linear high frequency transducer. Examination was performed in supine position with penis positioned along the anterior abdominal wall. Informed consent was obtained after explaining possible complications including Priapism.

Findings

B mode evaluation reveals normal appearance of bilateral cavernosal arteries.

No fibrotic or calcified plaque is seen. Baseline diameter and velocity of the cavernosal and the dorsal arteries were acquired and appear unremarkable.

Intracavernosal injection of 1.4 ml papaverine ( 2ml ampoule containing 30mg/ml) was performed under ultrasound guidance. Colour Doppler examination of cavernosal (CA) and deep dorsal (DDA) arteries was performed, 5 minutes interval for 20 min after intracavernosal injection of papaverine.

Cavernosal arteries show normal appearance.

Adequate rise is seen in the cavernosal arteries diameter after intracavernosal papaverine
injection.

Peak systolic velocity in right cavernosal artery was 42 cm/sec. While that in left was 35
cm/sec.


EDV after 20 mins was 5-6 cm/sec. Absent diastolic reversal noted.

Visual grading : E3

Impression
  • No arterial insufficiency.
  • EDV after 20 mins was 5-6 cm/sec suggestive venous leak.
